SMS Opt-In Form

This page documents the in-person SMS opt-in consent form used by Longan’s Place LLC. Prospective clients complete this form during a wedding-estimate consultation at our venue (15255 SW 200th Street, Miami, FL 33187) and check the SMS consent box to expressly opt in to recurring SMS communications related to their wedding estimate and event coordination.

By checking the SMS consent box on the form below, the client acknowledges that:

  • Message frequency varies based on their interaction with Longan’s Place LLC.
  • Message and data rates may apply.
  • They may opt out at any time by replying STOP, or reply HELP for assistance.
  • Mobile information will not be shared with third parties or affiliates for marketing or promotional purposes.
  • They have reviewed (or have been offered the opportunity to review) the SMS Terms and Conditions and Privacy Policy.
